This is not medical advice, and a stringer is not a clinician — but understanding the general shape of the evidence helps frame a useful conversation and know when to point a player toward a doctor instead of a tension chart.
What the general consensus supports
There is broad agreement among sports science and biomechanics sources that string bed stiffness affects how much shock is transmitted through the racquet to the arm, and that lower tension generally produces a softer, more shock-absorbing string bed than higher tension. This is treated as a reasonably well-supported relationship, not a fringe claim — it follows from basic material behavior rather than requiring a specialized study to establish. Equipment factors beyond tension, including frame stiffness, grip size, and string material, are also widely recognized as contributing to overall shock and vibration, often as much as or more than tension alone.
What is more contested or harder to isolate
Where the evidence gets thinner is in establishing how much any single factor — tension specifically — directly causes a condition like lateral epicondylitis, commonly known as tennis elbow, in an individual player. Injury causation in racquet sports is generally understood as multifactorial, involving technique, frequency and volume of play, prior injury history, and conditioning, alongside equipment. Isolating tension as a standalone cause in a real player, as opposed to a controlled lab setting, is difficult, and claims that frame a specific tension number as the fix for an existing injury should be treated with more skepticism than claims about general trends.

- A player with existing or recurring arm pain is generally a reasonable candidate for lower string tension as a precautionary adjustment, alongside other equipment changes such as a more flexible frame or a softer string material
- Tension adjustments are a sensible first conversation, but they are not a substitute for addressing technique or seeking a medical evaluation if pain is persistent or worsening

This protects against confusion on the next restring — a player who returns months later may not remember that their tension was lowered for arm comfort rather than chosen for performance, and a stringer working from notes rather than memory can keep the adjustment consistent without re-litigating the conversation each time. It also creates a simple record if the player later follows up with a doctor or physiotherapist and wants to describe what equipment changes they've already tried.
Equipment changes are worth trying, and tension is one of the easier and cheaper variables to adjust. But for anything beyond general comfort and prevention, a stringer's job is to point toward proper medical advice, not to diagnose or promise a fix that equipment alone can't reliably deliver.
